The Age Friendly Health System model is spreading rapidly throughout the country, with MHealth and the VA leading the way in Minnesota. This model focuses on four key issues for older people: What Matters, Mobility, Mentation, and Medications. This presentation will describe how clinicians across disciplines can assess and manage these issues. This presentation is part of the University's and MHealth's efforts to enhance care for older people. Dr. Ratner, Associate Professor of Medicine at the University of Minnesota, has been a leader in geriatrics medicine in Minnesota for over 30 years. He has applied principles of Age Friendly Health Systems clinically, in his research, and in program development. Continuing Medical Education credits are available.
