Objectives: Following the presentation, the attendees should be able to:
- Recognize the facilitators and barriers to implementing SDoH screening into a standardized comprehensive medication management (CMM) process in the setting of Federally Qualified Health Centers (FQHCs)
- Understand the impact of social determinants of health (SDoH) on the ability of patients to manage medications and on clinical outcomes in chronic conditions
- Identify opportunities for future studies and the expansion of SDoH screening in CMM practices
