Seminar Objectives: Following the presentation, the attendees should be able to:
Review background for new Centers for Medicare and Medicaid Services (CMS) value-based kidney care models
Identify opportunities for pharmacists providing medication management to work with nephrology practices to optimize medication management for patients with kidney disease
Introduce the Advancing Kidney Health through Optimal Medication Management Initiative and how it can help pharmacists take advantage of this new opportunity and obtain reimbursement for their services
